Upon submitting your request for a proof of claim form, please
understand that it may take up to 30 days to receive the form.
NEW CLAIM - If this is the first time
you are reporting a claim, please immediately submit, in writing, a detailed
description of your loss or accident. Include the
following documentation, if
available:
- Police report
- Summons & complaint
- Accord form
- Invoices/medical bills
- And any other information
We will then follow up with a proof of claim form to formally
recognize your claim in the estate.
