Welcome to the webpage for Family Health Network, Inc. On March 8, 2019 Family Health Network, Inc. and Community Care Alliance of Illinois, NFP were placed into rehabilitation. Dana Popish Severinghaus, Acting Director of Insurance of the State of Illinois, was affirmed as statutory and court-affirmed Rehabilitator. By Illinois law, the Director may appoint a Special Deputy Receiver to administer the receivership. The Office of the Special Deputy Receiver has been appointed to assist in the day-to-day administration of the rehabilitation proceedings under the direction of the Director.

On November 30, 2020, the Supervising Court entered an order approving the Rehabilitator’s application for authority to issue a 100% dividend on the 94 timely filed proofs of claim allowed at priority level (d) (policyholder level claims) of Illinois’ statutory schedule of distribution. Payment of the dividend will result in the distribution of $1,539,560.82 in Estate assets. It will leave approximately $3.2 million in unencumbered assets in the Estate to distribute on lower priority level claims allowed in the proceedings and to pay the continuing costs of administration.

On November 30, 2020, the Supervising Court approved a 100% dividend distribution on timely-filed and allowed claims at the policyholder priority level (d), of the Illinois statutory distribution scheme, in the amount of $1,539,560.82.

On June 19, 2019 the Court entered an Order Approving Deadlines and Procedures For the Filing of Claims against FHN assets. FHN insureds or policyholders (“Claimants”) have the right to present and file with the Liquidator a Proof of Claim Form on or before the Claim Filing Deadline of October 17, 2019 at 4:30 p.m. C.S.T. On June 7, 2019 the supervising Court entered an Order terminating the rehabilitation proceedings and closed the estate of Community Care Alliance of Illinois, NFP.

On April 22, 2019 the court supervising the rehabilitation proceedings entered an order approving an Agreement and Plan of Merger for the Sale of Community Care Alliance of Illinois, NFP (“CCAI”).
