On August 22, 2014, An Agreed Order of Liquidation with A Finding of Insolvency was entered against Concert Health Plan Insurance Company (“Concert Health”) by the Circuit Court of Cook County, Illinois pursuant to the provisions of Article XIII of the Illinois Insurance Code, 215 ILCS 5/187 et seq.

On November 17, 2015 the Supervising Court approved the Liquidator’s recommendation that a one hundred percent (100%) dividend be declared on all Concert Health Plan Insurance Company’s timely filed claims allowed and approved by the Court for distribution at statutory priority level (D) 215 ILCS 5/205(1)(d).
